Beberapa Orang Terlahir sebagai Pesimis

Kedengarannya menyedihkan, tetapi sepertinya ada di antara kita yang memang terlahir sebagai orang yang pesimistis.

Orang, yang secara genetis mudah memproduksi zat kimia otak yang mengontrol selera dan stres dalam jumlah sedikit, memiliki risiko lebih besar untuk terkena depresi berat, kata para peneliti.

Zat yang disebut sebagai molekul neuropeptida Y (NPY) tersebut mengatur respons otak terhadap rangsangan negatif dan respons terhadap rasa sakit.

Menurut peneliti dari University of Michigan, Amerika Serikat, temuan ini bisa menjadi diagnosis awal untuk mengatasi depresi dan penyakit kejiwaan lainnya.

"Kami berhasil mengidentifikasi sebuah penanda biologis—dalam hal ini variasi genetis—yang terkait dengan tingginya kasus depresi," kata Jon-Kar Zubieta, anggota tim peneliti, seperti dilansir Daily Mail.

Penelitian yang diterbitkan dalam jurnal Archives of General Psychiatry ini menggunakan tiga metode. Pertama, peneliti mengelompokkan subyek sukarelawan menjadi tiga kategori berdasarkan ekspresi NPY-nya: rendah, sedang, dan tinggi.

Lalu, menggunakan instrumen pencitraan resonansi magnetik (MRI), otak sukarelawan yang dipaparkan pada beberapa kata berbeda—ada yang bermakna netral (seperti benda), ada yang negatif (bunuh), dan ada yang positif (harapan) —dianalisis.

Hasilnya, grup sukarelawan dengan NPY rendah menunjukkan peningkatan aktivitas korteks prefrontal (bagian yang berkaitan dengan emosi), sementara grup dengan NPY tinggi menunjukkan respons lebih ringan.

Percobaan kedua adalah memberikan rasa sakit selama 20 menit dengan menggunakan cairan saline yang disuntikkan ke otot-otot rahang sukarelawan. Hasilnya sama: kelompok NPY rendah amat terpengaruh secara emosional sebelum dan sesudah rasa sakit.

BERBICARA PADA DIRI SENDIRI

Tanpa kita sadari, ternyata diri juga butuh untuk diajak bicara. Karena ternyata, dengan mengajak diri berbicara kita telah memberikan kesempatan untuk memperkaya diri.
Menurut para ahli, kebiasaan berbicara dengan diri sendiri atau self talk sudah ada sejak kita belajar bicara dan merupakan bagian dari proses belajar anak. Dan bicara dengan diri sendiri, tidak melulu dalam bentuk dialog dengan suara. Sebab semua yang terlintas dalam otak kita, sebenarnya cara kita berbicara dengan diri sendiri.
Maka jangan heran jika dalam satu menit, ada 150-300 kata. Itu artinya, 45-51 ribu kata per hari, total kata yang kita ucapkan setiap harinya pada diri kita sendiri. Menurut psikolog Toge Aprilianto dari Surabaya, berbicara pada diri sendiri bisa menjadi sangat dominan dalam diri setiap orang. Dan jika kita terbiasa berbicara pada diri sendiri akan memberikan banyak kelebihan, diantaranya:
1. Penyaluran emosi : Menggerutu karena lama menunggu adalah contoh dari berbicara dengan diri sendiri untuk menyalurkan emosi. Hanya saja, pastikan agar jangan sampai komunikasi dengan diri sendiri ini malah membuat kita semakin kesal.
2. Alat bantu mengambil keputusan : Semua faktor yang ada ketika kita dihadapkan pada dua kondisi untuk dipilih bisa dibicarakan pada diri sendiri dengan lebih fair.
3. Mengenal dan menerima diri sendiri : Berbicara dengan diri sendiri membuat kita lebih akrab dengan diri sendiri. Ini akan membantu kita tidak ikut terseret menjadi orang kebanyakan, karena kita lebih bisa mendengar kata hati kita ketika masuk dalam situasi atau lingkungan baru.
4. Berinteraksi dengan orang lain : Berbicara dengan diri sendiri akan menjadi media menimbang kebutuhan sendiri, mengira-ngira apakah orang lain bisa memenuhi kebutuhan kita, menimbang risikonya kalau ada untuk kemudian dijadikan referensi mengambil tindakan.
5. Memengaruhi orang lain dengan sehat: Sebelum menyakini orang lain, kita harus menyakinkan diri sendiri atau hal yang diinginkan dan direncanakan. Kalau kita tidak tahu apa yang kita mau, bagaimana bank akan percaya pada kita? Maka ajukanlah banyak pertanyaan kepada diri sendiri dulu, baru menyakinkan menyakinkan orang lain.
6. Mengembangkan diri : Berbicara dengan diri sendiri dapat menjadi media mengembangkan diri, jika kita menggunakan nada positif sama ketika kita menggunakannya untuk penyaluran emosi.

HITUNG JUMLAH RETIKULOSIT

Retikulosit adalah eritrosit muda yang sitoplasmanya mengandung sejumlah besar sisa-sisa ribosom dan RNA, dan berasal dari sisa inti dari bentuk penuh pendahulunya. Banyak retikulosit dalam darah tepi menggambarkan aktivitas eritropoiesis yang hampir akurat. Hitung retikulosit dinyatakan sebagai prosentase jumlah retikulosit per 1000 eritrosit. Nilai normalnya 0,5 - 1,5 %.

d. Cara Kerja

• ke dalam tabung reaksi kecil tetskan 2btetes larutan brilliant cresyl blue dalam natrium citrat atau new methylene blue.
• tembahkan 4 tetes darah, campurkan baik-baik dan biarkan selama15 menit agar pewarnaan sempurna
• inkubasi 37 derajat Celcius selama 15 menit, tabung tersebut kocok sekali lagi dan ambil setetes dengan pipit pasteur untuk dibuatn sediaan darah apus. Keringkan di udara dan periksalah dengan mikroskop.
• periksalah dengan perbesaran obyektif 100 kali. Carilah daerah yang cukup tipis di mana eritrosit tidak membuat reuleaux. Eritrosit biru muda dan retikulasit akan tumpak sebagai sel yang mengandung granula / filament biru. Bila kurang jelas bisa di counterstain (dicat lagi) dengan cat wright
• hitung dalam 1000 eritrosit dengan mempersempit lapangan pandang (letakkan kertas dengan lubang di dalam okuler)

HITUNG TROMBOSIT

Metode untuk menghitung trombosit telah banyak dibuat dan jumlahnya tergantung dari kenyataan bahwa sukar untuk menghitung sel-sel trombosit yang merupakan partikel kecil dan mudah aglutinasi maupun mudah pecah. Sukar untuk membedakan dengan kuman. Metode elektronik mempunyai ketelitian yang baik, tetapi metode visual yang langsung cukup dapat digunakan utnuk menghitung trombosit yang rendah sampai tinggi. Bila trombosit rendah, maka metode yang menggunakan plasma cukup bermakna. Metode elektronik tidak akan diuraikan.Yang akan dibahas dalam halaman ini adalah penghitungan trombosit dengan metode langsung (Rees-Ecker).

Darah diencerkan ke dalam larutan yang mengandung brilliant cresyl blue, sehingga trombosit tercat biru muda. Trombosit dihitung dengan bilik hitung. Hasil pemeriksaannya diperiksa ulang dengan sediaan apus.

d. Cara Kerja

hisaplah darah dengan pipet erotrosit sampai tanda 0,5 dan encerkan dengan larutan pengencer sampai tanda 101 (pengenceran 200x). Mulai saat ini trombosit harus selesai dihitung dalam waktu 30 menit, agar tidak terjadi disintegrasi sel-sel trombosit

kocoklah pipet tersebut 3-5 menit

bersihkan bilik hitung

siapkanlah cawan petri bagian dalam, dasar dan tutupnya ditempeli kapas yang ukurannya sama dan sebelumnya telah dibasahi air

setelah pipet tersebut dikocok, buanglah 4 tetes pertama dan tetesan ke lima isikan ke dalam bilik hitung. masukkan bilik hitung tersebut ke dalam cawan petri yang telah disiapkan tadi. Biarkan selama 15 menit , agar trombosit mengendap dan tidak terjadi penguapan.

letakkan bilik hitung di bawah mikroskop dengan perbesaran 100X obyektif dan kemudian perbesaran 40 x , trombosit tampak refraktif dan mengkilat berwarna biru muda / nila, lebih kecil dari eritrosit serta bentuk bulat, lonjong atau tersebar atau bergerombol.

jumlah trombosit =

rata-rata jumlah trombosit tiap kotak X pengenceran volume kotak trombosit(kotak kecil)

buatlah sediaan apus dengan pewarnaan Wright dan amati kesan jumlah trombosit sebagai periksa ulang terhadap metode di atas

PEMERIKSAAN HITUNG JUMLAH LEUKOSIT

        Hitung leukosit menyatakan jumlah sel-sel leukosit perliter darah (System International Units = SI unit) atau per satu mmk darah. Nilai normalnya 4000 - 11000 / mmk.Untuk penerapan hitung leukosit ada dua metode, manual dan elektronik. Pada umumnya metode elektronik belum digunakan secara umum, mungkin baru di laboratorium besar, sehingga cara manual masih memegang peranan penting. Metode elektronik tidak dibicarakan.

a. Dasar

Darah diencerkan dengan larutan asam lemah, yang menyebabkan sel-sel erotrosit hemolisis serta darah menjadi encer, sehingga sel-sel leukosit mudah dihitung.

              Sel darah putih, leukosit (en:white blood cell, WBC, leukocyte) adalah sel yang membentuk komponen darah. Sel darah putih ini berfungsi untuk membantu tubuh melawan berbagai penyakit infeksi sebagai bagian dari sistem kekebalan tubuh. Sel darah putih tidak berwarna, memiliki inti, dapat bergerak secara amoebeid, dan dapat menembus dinding kapiler / diapedesis. Dalam keadaan normalnya terkandung 4x10⁹ hingga 11x10⁹ sel darah putih di dalam seliter darah manusia dewasa yang sehat - sekitar 7000-25000 sel per tetes.Dalam setiap milimeter kubil darah terdapat 6000 sampai 10000(rata-rata 8000) sel darah putih .Dalam kasus leukemia, jumlahnya dapat meningkat hingga 50000 sel per tetes.

Filariasis adalah penyakit menular ( Penyakit Kaki Gajah ) yang disebabkan oleh cacing Filaria yang ditularkan oleh berbagai jenis nyamuk. Penyakit ini bersifat menahun ( kronis ) dan bila tidak mendapatkan pengobatan dapat menimbulkan cacat menetap berupa pembesaran kaki, lengan dan alat kelamin baik perempuan maupun laki-laki. Akibatnya penderita tidak dapat bekerja secara optimal bahkan hidupnya tergantung kepada orang lain sehingga memnjadi beban keluarga, masyarakat dan negara. Di Indonesia penyakit Kaki Gajah tersebar luas hampir di Seluruh propinsi. Berdasarkan laporan dari hasil survei pada tahun 2000 yang lalu tercatat sebanyak 1553 desa di 647 Puskesmas tersebar di 231 Kabupaten 26 Propinsi sebagai lokasi yang endemis, dengan jumlah kasus kronis 6233 orang. Hasil survai laboratorium, melalui pemeriksaan darah jari, rata-rata Mikrofilaria rate (Mf rate) 3,1 %, berarti sekitar 6 juta orang sudah terinfeksi cacing filaria dan sekitar 100 juta orang mempunyai resiko tinggi untuk ketularan karena nyamuk penularnya tersebar luas. Untuk memberantas penyakit ini sampai tuntas

WHO sudah menetapkan Kesepakatan Global ( The Global Goal of Elimination of Lymphatic Filariasis as a Public Health problem by The Year 2020 (. Program eliminasi dilaksanakan melalui pengobatan missal dengan DEC dan Albendazol setahun sekali selama 5 tahun dilokasi yang endemis dan perawatan kasus klinis baik yang akut maupun kronis untuk mencegah kecacatan dan mengurangi penderitanya. Indonesia akan melaksanakan eliminasi penyakit kaki gajah secara bertahap dimulai pada tahun 2002 di 5 kabupaten percontohan. Perluasan wilayah akan dilaksanakan setiap tahun. Penyebab penyakit kaki gajah adalah tiga spesies cacing filarial yaitu; Wucheria bancrofti, Brugia malayi dan Brugia timori. Vektor penular : Di Indonesia hingga saat ini telah diketahui ada 23 spesies nyamuk dari genus Anopheles, Culex, Mansonia, Aedes & Armigeres yang dapat berperan sebagai vector penular penyakit kaki gajah.

Cara Penularan :
Seseorang dapat tertular atau terinfeksi penyakit kaki gajah apabila orang tersebut digigit nyamuk yang infektif yaitu nyamuk yang mengandung larva stadium III ( L3 ). Nyamuk tersebut mendapat cacing filarial kecil ( mikrofilaria ) sewaktu menghisap darah penderita mengandung microfilaria atau binatang reservoir yang mengandung microfilaria. Siklus Penularan penyakit kaiki gajah ini melalui dua tahap, yaitu perkembangan dalam tubuh nyamuk ( vector ) dan tahap kedua perkembangan dalam tubuh manusia (hospes) dan reservoair.
Gejala klinis Filariais Akut adalah berupa ; Demam berulang-ulang selama 3 ? 5 hari, Demam dapat hilang bila istirahat dan muncul lagi setelah bekerja berat ; pembengkakan kelenjar getah bening (tanpa ada luka) didaerah lipatan paha, ketiap (lymphadenitis) yang tampak kemerahan, panas dan sakit ; radang saluran kelenjar getah bening yang terasa panas dan sakit yang menjalar dari pangkal kaki atau pangkal lengan kearah ujung (retrograde lymphangitis) ; filarial abses akibat seringnya menderita pembengkakan kelenjar getah bening, dapat pecah dan mengeluarkan nanah serta darah ; pembesaran tungkai, lengan, buah dada, buah zakar yang terlihat agak kemerahan dan terasa panas (early lymphodema). Gejal klinis yang kronis ; berupa pembesaran yang menetap (elephantiasis) pada tungkai, lengan, buah dada, buah zakar (elephantiasis skroti).

Diagnosis
Filariasis dapat ditegakkan secara Klinis ; yaitu bila seseorang tersangka Filariasis ditemukan tanda-tanda dan gejala akut ataupun kronis ; dengan pemeriksaan darah jari yang dilakukan mulai pukul 20.00 malam waktu setempat, seseorang dinyatakan sebagai penderita Filariasis, apabila dalam sediaan darah tebal ditemukan mikrofilaria. Pencegahan ; adalah dengan berusaha menghindarkan diri dari gigitan nyamuk vector ( mengurangi kontak dengan vector) misalnya dengan menggunakan kelambu bula akan sewaktu tidur, menutup ventilasi rumah dengan kasa nyamuk, menggunakan obat nyamuk semprot atau obat nyamuk baker, mengoles kulit dengan obat anti nyamuk, atau dengan cara memberantas nyamuk ; dengan membersihkan tanaman air pada rawa-rawa yang merupakan tempat perindukan nyamuk, menimbun, mengeringkan atau mengalirkan genangan air sebagai tempat perindukan nyamuk ; membersihkan semak-semak disekitar rumah.

Pengobatan :
secara massal dilakukan didaeah endemis dengan menggunakan obat Diethyl Carbamazine Citrate (DEC) dikombinasikan dengan Albenzol sekali setahun selama 5 ? 10 tahun, untuk mencegah reaksi samping seperti demam, diberikan Parasetamol ; dosis obat untuk sekali minum adalah, DEC 6 mg/kg/berat badan, Albenzol 400 mg albenzol (1 tablet ) ; pengobatan missal dihentikan apabila Mf rate sudah mencapai < 1 % ; secara individual / selektif; dilakukan pada kasus klinis, baik stadium dini maupun stadium lanjut, jenis dan obat tergantung dari keadaan kasus.
